Does the shed get demolished on site?
Yes, and it comes apart and loads at the same time. For anything larger than a small resin unit that is the only practical method — there is no version where a 10×12 wooden shed goes onto a trailer whole.
The order depends on the material. Wooden sheds come apart roof first, then panels, then framing, then floor decking and skids. Metal buildings unbolt, which is quick but produces sharp panels that need handling properly. Resin sheds mostly come apart by hand.
Occasionally a shed in genuinely good condition is worth moving rather than destroying, and where that looks possible you hear it before anything gets cut. It is not usually the case, but it is worth thirty seconds of looking.
What about the slab, the anchors and the footprint?
The shed comes off the slab and the slab stays, unless you specifically want it gone. That is the default and it is worth being clear about.
Concrete is heavy material, priced by weight rather than by volume, because that is how the transfer station charges for it. So a slab breakout is quoted separately from the shed itself. Say at booking whether the slab is going and it gets quoted as one job instead of becoming a second conversation later.

What happens with storm-damaged sheds?
